COVID-19 VACCINATION POLICY

HEALTH & SAFETY

Amended September 28, 2021
Policy Statement

Waterloo Wellington Flight Centre (the “Flight Centre”) is committed to the health and safety of
all students, employees, staff, contractors, and volunteers (“Community Members”) who attend
our facilities. This COVID-19 Vaccination Policy (this “Policy”) has been developed and
implemented in accordance with the direction from the Ministry of Colleges and Universities,
guidance from the Ontario Chief Medical Officer of Health, and local public health guidance and
directives.

The objective of this Policy is to reduce the transmission of COVID-19 to protect the health and
safety of the Flight Centre’s Community Members. To that end, and in accordance with
ministerial directives, the Flight Centre requires that all Community Members be fully vaccinated
against COVID-19, except as set out below.
This Policy comes into effect as of September 7, 2021.
Latest Amendment September 28 2021.

Proof of Vaccination
Upon arrival at the flight centre, Community Members must provide:
a) proof of vaccination against COVID-19; or
b) written proof of a medical reason (provided by a physician or a registered nurse) that
sets out a documented medical reason for not being fully vaccinated against COVID-19,
and the effective time period for the medical reason (i.e. time-limited or permanent).
Individuals with a Medical exemption must undergo an antigen test and demonstrate a
negative result at least once every seven (7) days. The official result paperwork of each
negative test must be shown upon entry to the flight centre.

For the time being, a Community Member may provide an attestation that they are vaccinated in
lieu of Proof of Vaccination; however, a Community Member who provides an attestation will be
expected to follow up with Proof of Vaccination by October 17, 2021.

In the future the Flight Centre may require Community Members to provide Proof of Vaccination
(or proof of a documented medical exemption) in electronic or paper format.

Prior to October 17, 2021, a Community Member who has not provided Attestation of
vaccination or Proof of Vaccination, including those who have provided proof of a documented
medical exemption, must undergo an antigen test and demonstrate a negative result at least
once every seven (7) days. The official result paperwork of each negative test must be shown
upon entry to the flight centre.

Rapid antigen testing is available on a walk-in basis at the Shopper’s Drug Mart located at 1005
Ottawa St N, Kitchener (519 894 6813) for $40. Antigen testing will be at the Community
Member’s expense.

All Community Members, other than those who require accommodation as set out below,
visiting the Flight Centre’s campus must be fully vaccinated and provide Proof of Vaccination by
no later than October 31, 2021.

Protective Measures
This Policy is but one step in the Flight Centre’s plan to protect the health and safety of
Community Members. Community Members are still required to undergo screening and wear
face coverings while indoors on campus and abide by all other local and provincial public health
guidance and directives.

Accommodation
The Flight Centre will provide accommodations for rare cases where community member cannot
be vaccinated for medical reasons or grounds protected by the Ontario Human Rights Code.
Accommodation requests will be assessed on a case-by-case basis.

Any Community Member requesting accommodation in relation to this Policy must disclose their
need for accommodation to Bob Connors (GM), provide supporting information/documentation
to substantiate the reason they are unable to receive a COVID-19 vaccine, and cooperate in the
Flight Centre’s efforts to accommodate.

This Policy will be interpreted and applied in a manner consistent with the applicable human
rights legislation.

Privacy
The Flight Centre will ensure that all information collected for the purposes of this Policy is used
only for the purposes of the Policy, shared only on a need-to-know basis, is stored securely, and
deleted when no longer required.

The Flight Centre is required to report non-identifiable statistical data on vaccination rates to the
Ministry of Colleges and Universities weekly.

Review and Modification
The Flight Centre will review this Policy on a regular basis and reserves the right to modify its
contents at any time, based on currently available public health information and
recommendations, any further directives or legislative amendments, and operational
requirements. You will be notified of any changes to this Policy.
